We request a 24 hour cancellation notice if at all possible.
Directions
From I-75 North/South: Take the Windy Hill exit and go west on Windy Hill six miles. Take a left onto Austell Rd. Go 2.9 miles and take a right onto Mulkey Rd. Our practice is located .3 miles on the left in the North Professional Building, Suite 200 (second floor).
Hospitalization
Our providers have privileges only at Cobb Hospital.
Insurance and Payments
Please be sure to bring both your Photo ID & updated Insurance card on the day of your appointment. Patients are financially responsible for their office visit and pre-payments.
