As a patient, it’s your right to get copies of most medical records. If you’ve received care at a Wellstar hospital or see a Wellstar doctor for ongoing care, you may access your medical records, by request, at any time.
We’ve done our part to make requesting your medical information easy. With your request—through MyChart, email, fax or mail—we'll provide you with doctor's notes, medical test results, lab reports and billing information.

Patients can immediately access provider notes, medical test results, lab reports, immunization records, billing information, and appointments through the MyChart Patient portal. If records are requested from the MyChart Patient Portal, you will receive them within 1 hour.
Request Your Medical Records
Our Patient
You can obtain copies of your medical records as a patient or patient representative. You could request your medical records anytime if you received medical attention at a Wellstar hospital or facility or ongoing care from a Wellstar provider. Requesting your medical records is easy and can be done through MyChart Patient Portal, online request, email, fax, or mail.
All other requesters (i.e., payors, disability, attorneys, third (3rd) party requesting firms)
Requests from insurance companies, attorneys, or Departments of Disability Determination Services, etc., should have a completed valid authorization and mailed to one of the addresses noted on our Authorization for Release Form [Spanish version]. Please note: If you are representing a deceased patient, we need valid authorization along with supporting documentation such as Healthcare POA, legal guardian, and executor of the estate. All charges for copies of medical records will be applied to the requester.
MyChart Patient Portal
Patients can immediately access provider notes, medical test results, lab reports, immunization records, billing information, and appointments through the MyChart Patient portal. Records not found in the MyChart Patient Portal may be requested and received through the portal. If you already have an active MyChart account with Wellstar Health System, request a copy of your records by Request Formal Copy of Health Record. Requested medical records can be accessed through the “document center” in MyChart. Once you get the PDF, you can save it to your personal computer. If you do not have an active MyChart account with Wellstar Health System:- On the MyChart main web page, click on Sign Up.
- Click No Activation Code, “Sign up online.”
- Complete the requested information and click Submit. You will receive an email with your activation code on activating your MyChart account.
- Once you log into MyChart, follow the previous steps to submit a request for your medical records.
Online Request
We offer an online records request option without signing up for a patient portal. The benefits of using online record requests would be a quicker response than mailing a request for processing. You must upload a copy of your photo ID, which can be taken via webcam or smartphone. Patient Representatives (healthcare POA, legal guardian, executive of an estate, etc.) must upload supporting documentation. This online tool will let you request all your records. As for patients and all other requesters, please print and fill out the Authorization for Release Form [Spanish version] and email, mail, or fax, along with a copy of your photo ID. Patient Representatives (Healthcare POA, legal guardian, estate executive, etc.) should include supporting documentation. Please note that physical mail is no longer received at Wellstar Atlanta Medical Center or Wellstar Atlanta Medical Center South. Please use the e-request or mail at other Wellstar locations to request records from these locations. You can go to any Wellstar location and request medical records if seen at multiple sites. On the actual form, please mark the areas requested on the release form. You can request and receive records from any location. The average turnaround time from the requestor is 7 - 10 business days from the time of receiving the form but may take up to 30 days. Additional Information to Know
- All requests for medical records must be fully completed and signed by the patient or patient’s representative.
- If the patient is under 18, a parent or legal guardian must sign the consent release.
- All legal guardians will be asked to provide documentation of legal guardianship.
- If a patient is deceased, the legally appointed executor of an estate or next of kin must consent to the release along with supporting documentation.
- After the properly executed authorization has been received, our team will review all required elements and processes by federal and state laws governing your privacy and medical records.
- Please allow 7 – 10 days for this process after receiving the request. In certain instances, it can take up to 30 days.
- For your convenience, we offer electronic delivery, or records can be mailed to the address listed on the authorization.

- Strongly encourage you to use our request form to expedite the processing of the records.
Cost for Paper and Electronic Copies of your Medical Record
- Record requested and delivered through the MyChart Patient Portal – No Charge
- Records are sent directly to another healthcare provider – No Charge
- Records delivered to the patient via email, mail, fax, or in-person – $6.50 plus tax
- Records are delivered to an insurance company, attorney, or third party. The applicable fee will be charged to the requester.
Records for Your Physician
If you have a provider/physician who is a Wellstar Health System affiliate or a provider/physician from outside of Wellstar, they may access your medical records from their office without you facilitating this request by creating an account using Wellstar Epic Care Link or by sending a fax request to us at 770-810-4193. Wellstar EPIC Care Link is a tool that provides real-time web access to patient information, so your provider can access clinical data and images with Wellstar to provide best-quality patient care. However, if your provider’s office has requested that you contact us and request your medical records from Wellstar Health System, we can fax the documents to your physician's office at no charge to you once you complete a request form in detail, ensuring the following items are included:- Patient Full Name/DOB
