Home-based palliative care is specialized medical care for people living with a serious illness who prefer care from the comforts of home. This type of care is focused on providing relief from the symptoms and stress of the illness. The goal is to improve the quality of life for both the patient and the family. Palliative care is based on the needs of the patient, not on the patient’s prognosis.
Palliative Care is provided by a specially trained team of nurse practitioners, social workers, care coordinators, nurses, and physicians. This team works together with a patient’s other doctors to provide an extra layer of support.
Who can benefit from Palliative Care?
- People with any serious illnesses, including cancer, chronic obstructive pulmonary disease (COPD), congestive heart failure (CHF), kidney disease, liver disease, dementia, HIV/AIDS, stroke, and many others.
- People with bothersome symptoms including pain, shortness of breath, nausea, vomiting, cough, confusion, trouble sleeping, anxiety, and depression.
- People and families who need help matching treatment choices to personal goals and values.
- People who have been seen by a Palliative Care provider in the past.
Does my insurance pay for Palliative Care?
Yes. Medicare, Medicaid, and most private insurances cover palliative care the same as any other medical specialty.Can I get Home Health at the same time as Palliative Care?
Yes. Home Health (Physical Therapy, Occupational Therapy, and Nursing support) services can be received at the same time as Palliative Care.Can I still pursue active treatment of my illness (i.e., hemodialysis, chemotherapy, radiation) if I receive Palliative Care?
