The health provider has her preconceived notions and does not listen to patient's concerns. She is refusing to fill out prior authorization form for my medication. My pharmacy has sent her a notification the same day to do a prior authorization but she is still waiting for a form from them which is not going to happen according to the pharmacy. I personally sent her this form on mychart and she is still refusing to fill it out. Several days before the appointment, I told her through mychart that she needed to fill out that form so we do not waste too much time before I get my medication. This is what most providers do but she refused to do it. This is very frustrating for me. Can someone else take over my care and send this form out?
