Dr. Carolyn Smallwood received her bachelor’s degree from Toccoa Falls College in Toccoa, Ga., and her master’s degree in public health from A.T. Still University in Kirksville, Miss. She obtained her medical degree from the Philadelphia College of Osteopathic Medicine in Suwanee, Ga. Dr. Smallwood completed her residency at Southern Family Medicine in Pueblo, Colo.
Dr. Smallwood is a member of the American Academy of Family Practice and the American Osteopathic Association. She is a diplomat of the American Board of Family Medicine.

Support staff at Dr. Smallwood's office failed to get a prescription called out on two different attempts to two different places. I wound up showing up at one of the places which was Costco and the prescription was not there even though it had been called in three days earlier and had to call back to the hospital again or back to the doctor's office again.
