Individual Care Plans
Personal care is a hallmark of Florence Hand Home. Each resident has an individual care plan established on admission and the plan is reviewed quarterly with revisions as needed. Residents and families are encouraged to meet routinely with interdisciplinary staff and review the resident plan of care.
Staff
Staffing ratios exceed all government required and recommended guidelines to ensure that all residents receive quality care in a timely manner including assistance with such everyday activities as dressing, eating, mobility, hygiene and grooming.
Physical, occupational and speech therapy services are provided through Wellstar West Georgia Medical Center with a physician’s order and are available onsite. A restorative nursing program provides additional follow up care when therapy may not be an option. Vision, dental and psychiatric services are available in house as needed.
Attending physicians make rounds at least monthly and are available to the nurse for consultation 24 hours a day, seven days a week. Residents may see other specialist with a consult from their attending physician. Recommended influenza and pneumococcal immunizations are provided on admission and annually per guidelines issued by the Centers for Disease Control and Prevention.
Services
Residents at Florence Hand Home enjoy housekeeping, linen service and personal laundry service as part of their monthly room and board fees. For the convenience of our residents, beautician/barber services are also available within the facility.
For those residents who make offsite doctor’s visits, Florence Hand Home provides transportation to and from scheduled appointments. Residents may also visit Walmart twice a month with transportation provided by the facility.
Activities
Activities and social time are an important part of healthy living. At Florence Hand Home, residents may participate in organized activities with staff and other residents. Family members are also encouraged to participate during special events throughout the year.
For those residents who enjoy getting away, there are offsite activities including fishing and picnics scheduled for their enjoyment. Many also attend worship services at area churches each Sunday.
There are also regular activities for those residents unable to leave their room. There are movies, talking books, and volunteers who visit to read to the residents. Bible study and worship services are also available throughout the week.
Designed with Safety in Mind
Florence Hand Home provides a comfortable environment for every patient. Double occupancy rooms are furnished and include private closet space for each resident. Bathrooms are handicapped accessible for wheelchairs and walkers.
Televisions and phones are located on each nursing unit and in the main lobby area. Private cable television and phone service is available at additional expense.
Designed with safety and security in mind, the facility includes sturdy yet comfortable furnishings, handrails in every hall, grab bars in all bathrooms and flooring conducive to safe walking. An outside patio welcomes residents and their visitors for those who want a breath of fresh air.
The Watchmate system, under continuous monitoring by staff, sounds an alert when patients begin to wander. Emergency preparation is also in place as Florence Hand Home is part of the Georgia Safe Net Program. Each staff member attends emergency preparedness education and drills are practiced monthly for fire and at least twice annually for disaster drills.
Menus and Food
Residents enjoy three nutritionally balanced meals a day, seven days a week. Nutritious snacks are also available throughout the day. Assistance is provided to those who need help eating. Accommodations are made for residents who may need special diets as ordered by their doctor.
Meals are served in the dining room and residents are encouraged to join their friends to socialize while enjoying their meals. For those unable to get out, tray service is provided as needed. Family members are always welcome to dine with their loved ones, with a required fee, and are encouraged to reserve space to join us for Thanksgiving and Christmas meals.
Admission Process
Our friendly staff provides assistance every step of the way for residents who decide to make Florence Hand their home. Potential residents receive a referral from hospital staff, family, friend, or other interested parties to begin the admissions process.
An initial assessment is conducted to evaluate the resident’s needs. Results of the assessment help in creation of a written care plan. Residents eligible for Medicare receive the initial assessment and then are reassessed after five days and 14 days, then again at 30, 60 and 90 days. Residents requiring long-term care receive quarterly assessments and can be assessed with a change in condition.
Residents receive a written statement that outlines resident rights and responsibilities upon admission. Residents also receive a detailed contractual agreement that clearly discloses healthcare, accommodations, personal care and supportive services; all fees, and admission and discharge provisions.
At Florence Hand Home, all beds are classified as skilled and are charged at a fixed rate. We participate in Medicare, Medicaid, Medigap, long-term care and private insurance programs.
Contracts may be terminated for non-payment. Consideration of refunds and/or transfers is handled on a case by case basis. Unfortunately, there are no private or corporate programs available to help cover the costs and Medicaid is the only government supported option.
Medicaid Information
Long-term care coverage is not provided by Medicare. A portion of a resident’s stay may be covered for a time after a qualifying hospital stay under the Medicare Skilled Nursing Benefit—however, this benefit is limited.
Most residents in the nursing home are covered by Nursing Home Medicaid. There is an application process that we will be glad to assist you with.
Some individuals pay private pay room rates for room and board. Please contact our business office for current room rates.
Long-term care insurance policies may cover a portion or all of the room charges.
