Disease Management
WellStar's Diabetes Disease Management program is a free self-management program for people who meet specific criteria. A registered nurse and licensed social worker act as health coaches to educate, encourage and support patients to manage their diabetes. Their role is to help people in their quest for optimal health through telephonic coaching. These calls help fill the gap between doctor visits. They preserve and support the patient/physician relationship and teach patients how to become active participants in their healthcare.
All WellStar employees and their dependents who are enrolled in the WellStar disease management may be eligible to receive reimbursement for diabetes medications co-payments.
